Attention all IAM members who may fall in the following categories

Layoff, Maternity Leave, Voluntary Leave, LTD,
Leave of absence not receiving a bi-weekly or weekly payroll check
or state disability and not receiving a bi-weekly or weekly payroll check

When this situation occurs it is the members responsibility to pay the
unemployment stamp fee of $2.00 per month to the local lodge for each
and every month the member is off payroll. This obligation extends to the extent
of the contract recall period, medical leave period, etc. Contact your shop steward
for your particular contract requirements.

Failure to pay the $2.00 unemployment stamp fee for two months can and
most likely will result in your membership lapsing. Should a member lapse
a $150.00 reinstatement fee will be required.

Please make checks payable to Local Lodge 1726. With your check
include a brief note stating your employer, classification and the
approximate duration of your leave if known. If you are off-payroll
for any other reason than the ones stated above and you are in doubt
as to your status contact the local lodge Financial Secretary for a clarification.
